Advanced Diagnostic and Treatment of Neuro Recovery Protocols for Traumatic Brain Injury - Day 1

This expanded two-day course taking TBI to the next level, will provide attendees with overwhelming information culled from peer-reviewed articles and Dr. Gordon’s practice and research on Traumatic Brain Injury as the causative factor for personality changes, cognitive impairment and mood disorders leading up to suicide. The attendees will be walked through the literature discussing the effects of primary trauma and the secondary effects of inflammation and oxidative stress on functionality. The world of peptides therapy is a growing field in medicine ranging from improving skin care, to growth hormone, to one’s immune system, to name a few. On Pub Med there are over 2.5 million articles on peptides. It is estimated that humans have over 250,000 peptides and we only understand a fraction of them. Peptides are chains of amino acids linked together by a polypeptide bond. The formation of a polypeptide bond by a dehydration synthesis will be reviewed. The difference between a peptide and a protein will be delineated. In addition the mechanism of action on how peptides work will be reviewed. The history of peptides will be covered and also knowing which hormones are peptides will be reviewed. Peptides have pleotropic effects and during the conference this subject will be addressed.

Growth hormone is an essential hormone in lowering body fat, improving on lipids, keeping muscles healthy and improving on bone density. Adult growth hormone deficiency symptoms will be reviewed and the association of higher mortality with growth hormone deficiency. Unfortunately the treatment with recombinant human growth hormone is not covered under most insurance plans, and it is cost prohibitive for most people. There is an alternative to the expensive recombinant human growth hormone most commonly used for boosting IGF-1 and it has the advantage of inducing all five of the hGH isoforms rather just the one that is found in the synthetic form. CJC-1295, Ipamorelin, GHRP-6, Teslomorelin and others are peptides that have been shown to help improve IGF-1 levels.

The thymosins are a group of peptides originally associated with the thymus gland. They possess pleiotropic effects on in humans. Thymosin Alpha 1 has long been studied for its immune enhancing properties, with regulatory approvals in the management of certain viral diseases and cancers. On the other hand, Thymosin Beta 4 has been shown to improve regeneration in multiple tissues. This lecture will cover the fundamentals of integrating both Thymosin Apha 1 and Thymosin Beta 4 into clinical practice. Dosing, indications and review of current literature will be presented.

BPC-157 was discovered in the 1990’s in gastric juice. However I give credit to Dr. Pavlov in Russia in the early 1900’s since he was manufacturing gastric juice from dogs to help heal many alignments. At that time they did not have the technology to detect BPC-157. BPC-157 has been studied extensively in animals and has been shown to improve on healing the body. Studies with BPC-157 have been shown to heal a severed tendon, nerve, and also ulcers. Although there are no human clinical trials the body builders has been using BPC-157 experimentally to repair their torn muscles, tendons and ligaments. In this lecture the published studies of BPC-157 ranging from colitis, inflammatory bowel disease, systemic steroid impaired muscle healing, bone defect, periodontitis, and others will be reviewed. Although the FDA has not approved this peptide, BPC-157 has been used worldwide.

An estimated 6 million Americans have Alzheimer’s disease, 10 million people worldwide with Parkinson’s, 3 million Americans suffer from traumatic brain injury (TBI), and approximately 800,000 will suffer from a stroke each year. There are peptides that have been shown to have neuroregenerative properties that have clinical studies in strokes, traumatic brain injury, Parkinson’s and in Alzheimer’s disease. There are many mechanisms that are involved with neurodegenerative diseases to traumatic brain injury. Inflammation in the brain, excessive oxidative stress, mitochondrial impairment, and excitotoxicity are common theme that can lead to damage or death of the neuron or myelin. Peptides have been shown to reduce inflammation, reduce oxidative stress, increase the production of brain derived neurotrophic factor and also increase the production of neurons. The peptides of Thymosin Beta 4, BPC-157, Cerebrolysin, Dihexa and Semax will be covered.

Alpha melatonin stimulating hormone analogues, including PT 141 and Melanotan, can act on multiple melanocortin receptors. As such, they have been studied and are currently being used for multiple indications, including increasing libido, altering skin pigmentation and modulating food intake. Epitalon is a pineal peptide, originally studied in Russia. It has been shown to activate telomerase, restore melatonin secretion and reduce mortality in users. This lecture will review both pineal and pituitary physiology as they relate to these peptides. Melanocortin receptors and neuropetides will be discussed. Dosing and protocols for PT 141, Melanotan and Epitalon will be highlighted.

Recognize when relevant Federal and State regulations are implicated within your age management practice and measure and evaluate necessary steps to ensure compliance with these statutes and regulations. This workshop will focus on one essential element; compliance as an ongoing process. Compliance with Federal and State regulations depend entirely upon a provider’s willingness to be motivated, alert, and self-critical. Compliance and compliance plans are a risk management tool that should address each and every area of potential exposure. Good faith efforts to focus on and implement thorough compliance mechanisms evidences a provider’s commitment to compliant and ethical practice and therefore undermines any allegation of intentional malfeasance. Compliance with Federal and State regulations is not only important when beginning a new practice, but is an essential element to the ongoing success of that practice. Continual monitoring of compliance “hot spots,” which requires a comprehensive understanding of the relevant regulatory framework, helps identify issues before they escalate into problems that could result in liability.

Obesity has gained epidemic proportions and is now the leading cause of chronic medical illnesses that occur as we age. Correcting obesity should therefore be paramount for every physician who practices age management medicine. Old concepts of weight-loss, such as low-fat diets, have been shown to actually increase cardiovascular risks. The Mediterranean diet has been proven to have beneficial effects on health but, per se, may not provide a good option for losing weight. Newer concepts such as higher fat diets, ketogenic diets and fasting mimicking diets have accumulated a plethora of evidence that suggests these appear to be preferred options for patients to lose weight. Physicians should consider this new evidence when recommending weight-loss options.

Over the past decade, the medical use of marijuana, CBD, THC, and other derivates of cannabis has increased exponentially in the U.S. Well over half the states have enacted laws in some form to allow physicians to prescribe marijuana medically—and by 2021, all 50 states may have laws allowing medical marijuana in some capacity. AMMG takes no political position on such uses—we are medical educators. That said, it’s clear a definite gap exists in medical education for physicians/healthcare professions who treat a rising patient base that is requesting or inquiring about medical marijuana for a gamut of disorders. AMMG stands in that gap to better educate and inform physicians so they are qualified to respond to this changing medical climate.

CBD (Cannabidiol) is emerging as a powerful supplement with a significant effect on our body’s endocannabinoid system. The endocannabinoid system has been shown to have a strong effect on the inflammatory, immune, cognitive and motor systems of the body. Cannabidiol (CBD) is one of over 80 cannabinoids, natural compounds found in industrial hemp. The endocannabinoid system (ECS) is perhaps the most important physiologic system involved in establishing and maintaining human health. Endocannabinoid receptors are found throughout the body: in the brain, organs, connective tissues, glands, and immune cells. In each tissue, the cannabinoid system performs different tasks, but the goal is always the same: homeostasis or balance. CBD has shown promising results in independent research and studies as a treatment for a variety of conditions including antiemetic, anticonvulsant, antipsyotropic, anti-inflammator, antioxidant, anti-tumoral, anti-anxiolitic/antidepressant, and more. We will review the basic science of CBD and the scientific evidence supporting the many applications of CBD along with the clinical applications of Full Spectrum Hemp Oil (CBD) that can be used in your medical practice.

Vitamin D’s reputation for helping bone porosity took a hit in JAMA, April 2018 from the U.S. Preventive Services Task Force. This independent group which makes health policy recommendations concluded that when it comes to preventing fractures, there is not enough evidence to recommend for or against supplementation for Vitamin D.  Because of the longevity of the boomers, osteoporotic fractures are an increasingly important cause of morbidity and mortality in the United States. Within 1 year of suffering a hip fracture, a high percentage of patients are unable to walk independently, more than half require assistance with activities of daily living, and 20% to 30% of patients will die. Approximately 2 million osteoporotic fractures occurred in the United States in 2005, and the annual incidence is projected to increase to more than 3 million fractures by 2025. This window of opportunity, which has been in plain sight, however, has been restricted from applying modern science to a botanical known as Cannabis sativa L. One of the compounds derived from this plant has been studied around the world as America has been in a prohibition for the last 39 years. The Farm Bill of 2018 will now allow us to study the benefits of the cannabinoid, cannabidiol, and its effects on bone density.

There is a lot more to cannabis than THC and CBD, its predominant cannabinoids. Terpenes are the fragrant oils that give cannabis its aromatic diversity. Flavonoids provide synthesis of antioxidant properties as potent inhibitors of cyclooxygenase enzymes and prostaglandin E2 production. The “entourage effect” identified by EB Russo, M.D. in 2011 offers medicinal properties beyond the efficacy of single cannabinoid effectiveness or a single CBD remedy for a specific disorder like seizure management. Whole plant medicine combines cannabinoids, terpenes and flavonoids to provide pain, sleep, gastro-intestinal, mood and behavior, and neurological benefits. The analysis of terpenes allows patients to determine correct strains for their symptom management. Of the more than 200 terpenes and 29 flavonoids found in cannabis, six primary terpenes and five flavonoids are key to selecting full spectrum cannabis products.

In 2002, the Women’s Health Initiative JAMA article was a “shot” which was heard around the world. There is one blockbuster treatment, that is by far the most cardio-protective treatment to date, but has been completely ignored by cardiologists and mainstream doctors. One recent article shows 81% reductions in all cardiovascular events (MI, revascularization, heart failure or death) in 43,000 men with established CVD who use PDE5 inhibitors regularly! There are even greater reductions in overall mortality if PDE5 inhibitors are combined with other treatments (statins and testosterone) in 857 diabetic men over 3.8 year periods – 60% mortality untreated vs <3% mortality if treated with all three treatments! Amazingly, there are other major benefits that include improved brain function and repair after stroke and possibly prevention for aging dementia. Additional major benefits include treatment of pulmonary hypertension and heart failure. One of the key mechanisms of action is the increased release of endothelial stem cells that populate the major organs. This effect may be the major reason for diverse health benefits way beyond the sexual benefits most doctors see as the major function of this category of drugs. The PDE5 inhibitors, like Viagra and Cialis, have a growing peer reviewed literature that outlines these many benefits. These references will be presented. I believe that the protective health benefits are the biggest breakthrough in medical history on CVD and other health protective benefits and are being ignored because of the limited focus on sexual benefits!

We will review the definition of Genetics, Genomics, and what Single Nucleotide Polymorphisms (SNPs) are and how they’re helping better the way we practice medicine. With the future of healthcare being personalized medicine, this technology will continue to help enable us to tailor medical care to the individual and maximize healthspan. SNPs, or our genetic variants, may predispose an individual to health risks but they too may prove beneficial. In reviewing some of the more well-studied SNPs we will dive into how to utilize this tool as part of a systems biology approach for advanced prevention. Additionally, in understanding patients’ metabolic needs, we may customize an ideal lifestyle such as exercise requirements, appropriate nutrition, and even alerting of potential food sensitivities which can cause occult inflammation. With an integrative approach we may help improve while even reverse conditions like IBS, migraines, and cardiovascular disease. One of the most dangerous enemies to men when it comes to boosting testosterone is the Aromatase enzyme. This destructive enzyme’s main role is to convert testosterone into the female hormone estrogen. It goes without saying that men who want to keep their testosterone at optimal levels need to take measures to keep this enzyme at reasonable levels because this will ensure that they are protecting testosterone and not allowing it to be converted into estrogen. Of course, we can control aromatase activity with the use of prescription medications, but what about the potential nutrient depletion and the side effects from the prescription drugs. There are “natural aromatase inhibitors” that have well documented effects without the side effects and have the potential for “side benefits” vs. side effects. We will review these natural aromatase inhibitors along with some of the other potential side benefits that these natural compounds process.

The fascia, more properly termed “fascial system”, builds a three-dimensional continuum of soft, collagen-containing, loose and dense fibrous connective tissue that permeates the body and enables all body systems to operate in an integrated manner. The basics of the fascial system at a microlevel (molecular and cellular responses) to the macrolevel (mechanical properties) will be reviewed to understand myofascial pain. Epidemiologic, clinical and experimental findings point to sex differences in myofascial pain, where women tend to experience pain more than men. Newer research has found there is hormone receptor expression in human fascial tissue; specifically, relaxin receptor 1 (RXFP1) and estrogen receptor-alpha (ER alpha). It is theorized that one of the stimuli for sensitization of fascial nociceptors could come from the hormonal factors estrogen and relaxin that are involved in extracellular matrix and collagen remodeling. Thus, it would follow that bioidentical hormone replacement therapy (BHRT) would contribute to restoring baseline function to aging myofascial tissue in the perimenopausal/ menopausal woman. This discussion will review current understanding of the myofascial components of pain transmission in the perimenopause and menopause stage of a woman’s life and provide an approach to bioidentical hormone replacement therapy to address these concerns.

This presentation is based on research published in 1980 by the Endocrine-Metabolic Service of Walter Reed Army Hospital, which told us that iodine and lithium rapidly controls thyrotoxicosis. The presentation provides detailed case examples including supplementation amounts, testing, and other recommendations until resolution of condition. The next segment of the lecture discusses the relationship of elevated rT3 with toxic metals and how to recognize the problem. The final section deals with TSH, investigating whether or not TSH should be relied on as an indication of thyroid function. Discussion on the relationship between TSH and TRH, and a few of the other non-thyroidal functions of TRH are provided.

Gut dysbiosis is associated with inducing a chronic inflammatory state that affects every organ and is implicated in the pathogenesis of multiple chronic conditions in the scope of internal medicine. Here we articulate relevance, diagnostic/therapeutic concerns of translational mucosal immunology (“Enteroimmunology”) and articulate the impact on measured cortison secretion patterns as demonstrated by 24 hour salivary cortisol testing. Inflammatory cytokines stimulate cortisol secretion, and over time, an endocrinopathy develops: dysfunctional diurnal cortisol secretion, depletion of beneficial hormones such as testosterone, DHEA, progesterone, pregnenolone, thyroid hormone, and sometimes estrogens (although elevations of estrogens due to impaired excretion is more common) occur. These physiologic events are implicit in the process of aging. We will describe treatment approach and cases featuring gut microbiome repair and restitution of optimal hormone levels as essential to lessening physiologic/cellular aging and inflammation.

In this presentation, I will delve deep into the connections between: Insulin resistance is becoming epidemic; Type 2 diabetes is on the rise; LADA – latent autoimmune disease of adulthood is common, and very much overlooked by a lot of medical practitioners. Insulin resistance and inflammation, the production of auto-antibodies and the effect on blood sugar control, and how all of the complications can be prevented by early detection of insulin resistance – what I term pre-insulin resistance. “Pre-insulin resistance” can be detected decades before it is clinically recognized, and as nutrition practitioners, we are uniquely positioned to coach our clients on how to prevent, detect and reverse it naturally.

Alzheimer’s Disease is a neurodegenerative disorder – but it is not a mysterious, untreatable brain disorder. It is a reversible multisystem illness that occurs due to toxins and infections and inflammation and lack of hormones and other diet and lifestyle factors. It has a strong autoimmune component as well. Neurodegeneration does not just happen. It happens for reasons, and when we can look at the big picture and address all of the driving factors, we can gain traction in stopping and sometimes even reversing the degenerative process. Alzheimer’s Dementia is a multi-factorial disease, and it is rarely due to just one thing. In order to reverse dementia, we have to look at, and rectify, all of the factors that contribute: metabolic, nutritional, toxic, inflammatory, infectious, autoimmune, and/or withdrawal of trophic support (like hormones and nutrients). Dr. Toups will discuss the Functional Medicine approach she used to reverse her own dementia, and how to evaluate patients with Mild Cognitive Impairment and Dementia using a Functional approach to determine the root causes of their illness.

A multifaceted approach described by Dr. Dale Bredesen was used as guiding methodology. We describe the population demographics, functional parameters including glucose regulation, intestinal barrier function, key nutrient and hormone levels, biotoxin impact, and chronic infections. We include dropouts from the program, describe our intervention, and report on cognitive outcome where available.

Brain MRI imaging with computerized assessment of the volumetric measurements of multiple brain regions is routinely used for diagnosing Alzheimer’s Age-related Atrophy in the Bredesen protocol. Other uses are for Traumatic Brain Injury, changes related to environmental toxins such as mold and Lyme, and limbic dysregulation. The MRI is easily obtained at most radiology facilities and does not use radiation or require contrast. Most insurances will cover the cost with proper coding. This lecture will cover the basics in learning to interpret the reports in a way that patients can know if their aging brain is showing changes consistent with Alzheimer’s. Other cause of cognitive impairment including sequalae of traumatic brain injury, neuroinflammation and other brain regions showing neurodegeneration such as the frontal lobes and basal ganglia will be covered. Multiple treatment interventions will be discussed.
