Practice Management Workshop: Launching and Growing a Successful Age Management Practice

Many attendees of the AMMG conferences are attracted to the field of Age Management Medicine, but do not feel qualified, or empowered with the necessary tools, to successfully transition from the well-established structure of disease-based medicine or from the traditional insurance-based reimbursement system into a system which relies to a significant degree upon direct pay. In addition, this workshop is also designed to complement the AMMG Certification in Age Management Medicine for physicians and healthcare professionals and to help educate and inform medical and office staff. Attendees will be guided through the process of developing an Age Management Practice. Our goal is for the attendee to be equipped with the basic tools for making the necessary decisions on what is required from a Practice Management point of view.

Aside from routinely prescribing testosterone for men, it is important to understand many basic concepts before doing so. Wading through disagreements and issues with peers can also become problematic if you are not well-versed in the literature and controversies. Prescribing hormones for men can be simple; however, dealing with side effects and complications is not. This lecture will address common problems, complications, side effects, monitoring, understanding levels, erythrocytosis vs. polycythemia, prostate cancer and elevated PSA, use of 5-alpha reductase inhibitors, and aromatase inhibitors and the harm of blocking DHT and estrogen. Prescribing testosterone to men is easy 90% of the time. However, it is the other 10% that requires skill, experience, insight, and command of the medical literature to appropriately treat routine and confusing cases. This lecture will review many usual as well as unusual cases and scenarios that will enable the experienced clinician to handle those difficult cases. A literature review will be provided to support why we do what we do in various scenarios.

Contrary to the standard RDA for nutrients, various factors that contribute to a person’s actual needs include: decreased ability to absorb nutrients, medications that may deplete the body of certain nutrients, and environmental factors that contribute to a greater need. These include: HIPAA/HITECH privacy and security requirements, including enforcement initiatives of the Office of Civil Rights; and Corporate Compliance issues, including OIG enforcement of the Stark law, the Anti-kickback Statute, and relevant state-based statutes and regulations. The focus of this workshop will be risk management and liability avoidance through internal policy development, self-auditing, and other proactive risk prevention strategies. The Stark law, the Anti-kickback statute, and the Anti-Inducement provisions of HIPAA are a source of confusion for most lawyers, lets alone physicians. This seminar will provide a summary of each law – and their state-based equivalents – in plain English. The workshop will focus on one essential element; compliance as an ongoing process. Compliance with Federal and State regulations depend entirely upon a provider’s willingness to be motivated, alert, and self-critical. Compliance and compliance plans are a risk management tool that should address each and every area of potential exposure. Good faith efforts to focus on and implement thorough compliance mechanisms evidences a provider’s commitment to compliant and ethical practice and therefore undermines any allegation of intentional malfeasance. Compliance with Federal and State regulations is not only important when beginning a new practice, but is an essential element to the ongoing success of that practice. Continual monitoring of compliance “hot spots,” which requires a comprehensive understanding of the relevant regulatory framework, helps identify issues before they escalate into problems that could result in liability.

Obesity has gained epidemic proportions and is now the leading cause of chronic medical illnesses that occur as we age. Correcting obesity should therefore be paramount for every physician who practices age management medicine. Old concepts of weight loss, such as low-fat diets, have been shown to actually increase cardiovascular risks. The Mediterranean diet has been proven to have beneficial effects on health but, per se, may not provide a good option for losing weight. Newer concepts such as higher fat diets, ketogenic diets and fasting mimicking diets have accumulated a plethora of evidence that suggests these appear to be preferred options for patients to lose weight. Physicians should consider this new evidence when recommending weight-loss options.

Workshop: Genomics as a Clinically Usable and Relevant Tool for Improving Healthy Aging

Genomics can be a powerful tool for precision medicine and age management. Chronic diseases associated with aging such as cognitive decline, heart disease, macular degeneration and osteoporosis are not caused by one gene. They are instead caused by the interactions of multiple small genomic variants interacting with the diet, lifestyle, environment and each other. Genomics can provide a lens into how to optimize health and longevity. Part of this lens is identifying genomic contributing factors to chronic diseases and using the understanding of gene function to address and optimize their management. In this workshop participants will learn how to make genomics clinically actionable so that better brain, cardiac and overall health and longevity can be obtained for your patients. Even if you are not sure if you want to use genomics in your practice this workshop will benefit you by giving you a deeper understanding of contributing factors to chronic diseases and how to address them!

Workshop: Essential Knowledge in the Clinical Use of Peptides for Age Management Medicine

The use of Peptides and Peptide Therapies has experienced exponential growth over the past few years. Their uses range from impacting hormonal change, to antibiotic properties, to treatments for conditions of aging such as cancer, cardiovascular health, memory & brain health and sexual function. AMMG in conjunction with the Clinical Peptide Society have an educational mandate to fill knowledge gaps for physicians and healthcare professionals. This course is a focused track on discovering the power of peptides and the essential knowledge and medical science in the clinical use of peptides for Age Management with special attention to legalities & FDA approved and non-approved compounds and uses. At the conclusion of this workshop the Clinical Peptide Society will offer attendees the opportunity to take an online examination on this material and obtain a Certificate of Completion for this course.

This session will address the basics of what a peptide is, what receptor does it interact with, how do peptides work, how many peptides do we have and which hormones are peptides. In addition, how a peptide is synthesized in a lab will be reviewed. The benefits of peptides and where most of the peptides that can be obtained will be covered. (Although as of May 21, 2023 using peptides in California is prohibited, so we don’t know how one in California can obtain them.)

To achieve their potential therapeutic benefits, agents designed for intra-articular use must be injected directly into the knee joint space and not into the anterior fat pad or the sub-synovial tissues. In our study, the accuracy of needle placement was assessed in a prospective series of 231 consecutive injections in patients without clinical knee effusion. Accuracy rates for needle placement were confirmed by an observer with fluoroscopic imaging to document the dispersion pattern of injected contrast material. The results showed that of 231 injections performed through an anterolateral portal, one hundred eighty were confirmed to have been placed in the intra-articular space on the first attempt, with an accuracy rate of 77.9%. All physicians should use fluoroscopic confirmation of placement when injecting agents into the intraarticular space of the knee.

What exactly is longevity? Merriam-Webster Dictionary defines it as the “long duration of individual life.” The term comes from the Latin word longaevitās. In this word, you can see how the words longus (long) and aevum (age) combine into a concept that means an individual who lives a long time. The goal is to strive for our maximum potential as we age. Longevity implies liver longer than the average lifespan.Longevity medicine is personalized, preventative, proactive and precision medicine that is powered by in-depth biomarkers of aging. It is a fast-emerging field in integrative/functional medicine and is becoming mainstream in the Western world. Ayurveda has long been associated with finding the root cause and healing the mind-body-spirit with natural methods. The science of biomarkers for aging is central to longevity medicine. Looking at genetic expression and the microbiome for example are tools to gauge cellular senescence. Senescence is defined as biological aging and the gradual deterioration of functional characteristics in a living organism. Senolytics or geroprotectors are substances that are used to halt or reverse cellular aging. Scientific studies have shown there are certain nutrients and nutraceuticals that can achieve this feat. Following my four pillars of a healthy lifestyle lays the foundation for promoting longevity. These include clean diet, exercise, restorative sleep and stress reduction. Maintaining homeostasis withing the biological systems is the key to anti-aging and protecting cellular destruction which is a byproduct of chronic inflammation. The Invitro and Invivo Development of a GLP-1 Stimulating Nutraceutical Taking a Full Pharmacological Approach

Nutraceuticals bridge the gap between nutrition and pharmacology, offering potentially effective health treatments to those either unwilling or unable to take pharmaceuticals. To data, the prospect of an impactful nutraceutical market is widely unrealized, with the presence of many unproven or ineffective products leading to consumer and clinician skepticism. In 2010 Plant and Food Research began a 20M NZD government funded research program to develop and scientifically validate a nutraceutical-based appetite suppressant for use during weight management. This ongoing research has spanned the last 13 years, combining nutritional science with drug discovery, and has developed a novel nutraceutical that stimulates the release of GLP-1 by targeting bitter taste systems present within enteroendocrine cells of the gastrointestinal (GI) tract. This talk will discuss the approach taken to develop this appetite suppressant. It will include how traditional medicine informed the identification of bitter taste as the biological target, the validation of bitter taste systems within appetite-regulating cells of the GI tract, and the clinical interventions demonstrating efficacy for both the stimulation of appetite suppressing GI hormones, GLP-1, CCK and PYY, as well as its use in reducing hunger and rebound eating during an acute model of fasting. Have you ever wondered how long you’re going to live? The potential answer can be found in the energy-producing cellular powerhouses called mitochondria. According to a growing number of cell biologists, the number and functionality of the mitochondria specifically determine an individual’s life span. When we are young, we are relatively protected against mitochondrial deterioration. As we age, however, changes within our cells lead to the destruction of mitochondria—paving the way for aging and disease. Mitochondria become damaged by various mechanisms during our lifetime, such as mutations in their DNA when they divide, by free radicals, and  epigenetic changes. When mitochondria are damaged, cells don’t retain enough energy to properly function and maintain themselves. Also, damaged mitochondria send signals to the cell, further disrupting proper cellular functioning. Damaged mitochondrial DNA becomes permanent a decade after mitochondrial dysfunction begins but in the early stages this damage remains potentially reversable.  There are various natural substances along with proper diet and lifestyle that can improve mitochondrial health. All these changes must be implemented early so that permanent damage does not accelerate the aging process.
